Trauma 3 was an upcharge haunted maze that was featured during Halloween Horror Nights 2017 (Japan). It was located in Stage 18.

Endless despair. After arriving at the facility, you must now embark on a long, mad journey. You’ll witness twisted human experiments that have been covered up, and uncanny creatures that attack you no matter where you go... A ghastly new maze opens its doors: the longest in the park’s history, where the wails and screams of terror never cease.

The whole experienced lasted 35 minutes and is the longest attraction in Universal Studios Japan so far. It took place within a multi-storey complex that had been transformed into a hospital. Full of many winding hallways and jump scares with shocking twists and with situations that will split groups apart to take part in their own experiences.

Some memorable scenes featured in this maze were:
- Getting "chained up" together in groups of 4-6.
- Walking down dark corridors with a faulty flashlight.
- Removing live roaches from a corpse.
- Reaching blindly into an enclosed box and pulling out gooey body parts.
- Seeing someone's arm getting chopped off.
- Getting chased around by crazy people.
- Walking down a scary hallway of doors lined with surgeons.
- Ordered to leave your friends behind and go inside individual cells.
- Strapped down to a bed in one of the cells and surviving moments of horror alone.
- Fending for your life alone inside a large maze.
- Closing a large gate to prevent "creatures" from chasing guest further ensuring their safe escape.
